Could it be that (over only 50 miles?) the tappet managed to move up while also turning around its axis so as to eventually tap the screw thread in the cover?

Michael is likely correct... And if that's the case, the tappet screw (but not the lock nut) screwed its way out of the cover.

Thanks for the reply's. That's my working theory: that the locking nut vibrated off and disappeared into the bowels of the engine and the tappet slowly unscrewed, pressed against the inside of the cap and banged a hole and slowly tapped a threaded hole during its escape.
