CB77 Exhaust Sleeve

Has anyone experienced blow out of their CB77 exhaust sleeve? Thanks lm for the history and education lesson on the sleeve I really appreciate it.

I assume going forward with the fix that I will have to purchase p/n 18391-259-000 again.
